Insurance Information
We accept most major insurance plans.
Appointments
Please call the office to make an appointment in advance. If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery. Cancellations for appointments require 24 hours advance notice. There is a $50 fee for not cancelling an appointment. Please arrive 15 minutes before appointments to fill out any new information. Remember to bring your insurance card, and let the receptionist know of any changes to your address, phone number, or any other pertinent information.
Fees & Payments
We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. We accept cash, checks, Visa, Mastercard, and American Express for your convenience. If we are a participating provider of your insurance company, we will bill them. However, payment is the patient's responsibility. We will help in any way we can to assist you in handling claims.
Prescriptions & Renewals
Please request all prescriptions and authorizations for renewals during our regular business hours when our full records are available. Renewals requested at other times will be filled only for extreme or emergent circumstances. We will not refill any prescriptions over the phone, if you are overdue for your office visit.
Office Policies
- Kindly notify us in writing if you would like us to discuss your health with another person.
- If you do not hear from the office within one week of a test performed, it is your responsibility to contact us.
- Reproduction of medical records cost $1 per page. Please allow 3 - 5 days to process medical records.
- We are unable to call anyone with a caller ID block on. You must remove your Caller ID block to receive a call back.
- Our practice is oriented in an evidence based medicine model with a progressive approach to health care delivery. We hope that you will find us approachable and knowledgeable.
- We will gladly release any X-ray to be used for further diagnostic testing procedures, other consultations
- Pre-certifications for testing may take up to 3 - 5 business days. Patients must contact us to begin the process.
- Any patient under the age of 18 must be accompanied by a parent or adult guardian who has written parental permission.
- To be compliant with HIPAA regulations, it is necessary that you and your children remain in your room with the door closed.
- Children under the age of 3 or those with special needs may require adult supervision during an X-ray. No minors or pregnant females will be allowed in the X-ray room. By law, our staff is not permitted to hold your child. Please make arrangements as necessary.
- We advise you to schedule your follow-up appointment before leaving our office. This will better assure you the date and time you require.
